SUMMARY: The Case Manager IV provides assessment, monitoring, planning, linkage and advocacy for the most appropriate services to individuals enrolled in the PH-RRH/PSH. Qualified candidate must possess a clear understanding of the service delivery system and the resources available for individuals, Veterans, Non-Veterans and their families. Individual will effectively maintain and manage assigned caseload contingent on needs, strengths, abilities, preferences of the individual served, and the program to which the individual is assigned.
ESSENTIAL DUTIES AND RESPONSIBILITIES: (These essential job functions are not to be construed as a complete statement of all duties performed. Provide crisis intervention to address the immediate needs of the very low-income, chronic and non-chronic homeless population
Assess the individual's strengths, needs, abilities and preferences to assist in the development of housing goals
Enforce program rules and procedures to ensure compliance with all government and contract regulations
Assist clients with varying needs (i.e. general information, veteran assistance, and referrals)
Facilitate the move to transitional and/or permanent, independent housing, and coordinate service when appropriate.
Provides effective assessments, therapeutic strategies along with crisis management
Knowledge of harm reduction, motivational interviewing and trauma-informed care.
Post information relative to job skills, housing, social events on the bulletin board-provide different resources to meet the clients need
Set attainable goals and monitor progress of clients, find the barriers and ways to remove them to attain self sufficiency
Provide and monitor the ongoing provision of services from community agencies and maintain the case management and provider agency information current with the progress of the individual.
Serve as a liaison to community agencies, network with community providers and seek out new services available to the clients.
Post information relative to job skills, housing, social events on the bulletin board-provide different resources to meet the clients need
Create programs at PSH/PH that will be appropriate for both RRH and PSH clients
Conduct home visits

Process Temporary Financial Assistance
Utilize Case Worthy, HSMIS for data collection/case record
Organize and conduct client satisfaction surveys and perform other duties relative to the position including typing, filing, answering telephones, etc.

Advanced knowledge of homelessness, severe and persistent mental illness and substance abuse
Advanced knowledge of resources in the community available for veteran population, especially services and programs offered by the VA
Ability to form partnerships in the community and seek out community resources
Must be able to comply with complex governmental regulations, policies and procedures and demonstrate thorough document compliance efforts and activities
Must be proficient with data management and information systems and have basic knowledge of Excel, PowerPoint, and Outlook
Must demonstrate excellent interpersonal skills and possess the ability to interact effectively with other agencies and service providers
Candidate must be able to work in a fast-paced environment and understand the issues that are faced by low-income populations
EDUCATION AND EXPERIENCE: (Pending on position and if prior to hire is approved by Chief Executive Officer, a comparable amount of training, education or experience may be substituted for the minimum education qualifications)
Masters degree and/or experience 5-years’ experience (work, life or volunteer) in social services.
Veterans preferred
GENERAL PHYSICAL DEMANDS : These physical demands are representative of the physical requirements necessary for an employee to successfully perform the essential functions of the job. WORK ENVIRONMENT: Environment will occasionally become noisy due to equipment operations and interactions among clients and staff. There may be the possibility of being exposed to communicable disease, possible exposure to verbal abuse or similar behavior from residents/clients. On an as needed basis, employees may be called upon to work outside of the established work schedule or work odd hours. All information associated with the Agency is confidential.
