POSITION SUMMARY:
The Clinical Documentation Quality Specialist is responsible for providing comprehensive training, support, and education to all clinical staff within Home Care and Hospice on documentation practices, CMS mandated assessments such as OASIS (Outcome and Assessment Information Set)/ HOPE), and clinical compliance. This role ensures adherence to all federal, state, and local regulations, including the Conditions of Participation (CoPs) for home care and hospice. The Clinical Documentation Quality Specialist will work closely with the clinical and quality team to enhance their documentation skills, ensure accurate and timely completion of documentation, and maintain the highest standards of clinical practice.
EDUCATION:
Associate of Science in Nursing required. BSN strongly preferred. State of NJ license or legal permission to work from the NJ Board of Nursing required. CPR certification required. OASIS Certification or Hospice Certification required within one year of hire.
EXPERIENCE:
Minimum of 2 years of experience in home care or hospice setting. Demonstrated expertise in clinical documentation, OASIS/HOPE, and regulatory compliance. Previous experience in education or training preferred. Computer and keyboard skills required, including PowerPoint, Excel and Word.
