Overview
West Pines Behavioral Hospital is a new 144-bed inpatient behavioral health facility serving Denver area residents located at 11455 Huron Street, Westminster, CO. A joint venture between Intermountain Health and Acadia Healthcare. Just opened in December 2024, the hospital provides comprehensive inpatient and intensive outpatient services to address the growing need for accessible, high-quality behavioral health care in the Denver metro area.

Pay range: $25.00- $28.00

Responsibilities
​ ESSENTIAL FUNCTIONS:

​Develop continuing after care plan, based on patient's needs, to promote long term health and relapse prevention.

​Responsible for setting discharge appointments for patients, including group or individual therapy, psychologist or psychiatrist.

​Make arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care.

​Act as a liaison between referral sources, patients and counselors.

​Ensure all patient discharge planning documentation is clear, concise and timely.

​ OTHER FUNCTIONS:

​Perform other functions and tasks as assigned.

Qualifications
​ EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

​Associates degree in nursing or human services field required. Bachelor's degree in social work or human services field preferred.

​One or more years’ experience in a clinical setting preferred.

​One or more years’ experience in discharge planning preferred.

Preferred Qualifications:

Previous experience in discharge planning, care coordination, case management, behavioral health, social services, or a related healthcare setting preferred.

Knowledge of community resources, behavioral health services, and levels of care preferred.

Experience coordinating referrals, aftercare appointments, and community-based services preferred.

Familiarity with psychiatric, substance use, or inpatient healthcare settings preferred.

Strong organizational, communication, and interpersonal skills

Ability to establish effective working relationships with patients, families, providers, and community partners.

​ LICENSES/DESIGNATIONS/CERTIFICATIONS:

​If applicable, current licensure as a LPN, RN within the state where the facility provides services.

​CPR and de-escalation and restraint certification required (training available upon hire and offered by facility.

​First aid may be required based on state or facility requirements.
