Purpose
Under the direction of the Manager of Shelter Operations, the Shelter Case Management Aide (CM Aide) provides supervision, safety, organization, and program management support to all clients that reside at the main shelter. The CM Aide networks with the Greater New Haven Recovery Focused Programs, Sober Living Programs and Social Service System and collaborates with other Columbus House residential staff to provide an environment that reflects the core values of Trauma Informed Service: Safety, Trustworthiness, Choice, Collaboration, and empowerment.
Duties and Responsibilities
Perform regular wellness rounds at the main shelter
Supervise up to 60 clients to ensure compliance with all house rules
Support and engage clients to assist them with following their service plans and achieving their goals
Observe and document medication calls when assigned
Help move client belongings as necessary
Complete work orders when needed
Collaborate with other agencies and social service providers
Attend agency and program meetings as scheduled
Complete all required reports/incident logs/ case notes/incident reports
Enforce house rules and maintain incident log
Supervise resident chores
Be proficient with CT HMIS system and enter data as directed
Support Manager by completing pre-screenings, intake/admission, case notes, and discharges and collaborate on individual service plan creation and execution.
Remain on-site to perform essential functions during emergency situations as designated by the Director of Emergency Services
Other related duties as assigned
Qualifications
High School Graduate or equivalent
Demonstrated the ability to manage, motivate and direct clients.
Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics
Knowledgeable of basic security techniques and concerns
Culturally competent; must possess the capacity to work within a team and foster teamwork.
General understanding of crisis intervention skills, confidentiality, boundaries, universal precautions
Strong empathic skills coupled with a high degree of reliability and punctuality.
Able to effectively use the local network of substance abuse, mental health, and social service providers.
Able to meet all documentation requirements including pre-screenings, intake/admission, case notes, discharges etc.
Able to foster a Harm Reduction philosophy within the work environment.
Valid Drivers’ license with a good driving history
Supervised by: Director of Emergency Services Salary: $19 per hour plus excellent benefits Schedule: Monday – Friday 8am-4pm
