The Lotus Case Manager/Intake Coordinator is responsible for performing a variety of duties within case management, utilization review, and community correspondence in the inpatient setting. These include, but are not limited to, daily contacts with Lotus referral sources and Third-Party payers, discharge planning and aftercare coordination. The Lotus Case Manager/Intake Coordinator will be a liaison between the acute and Lotus adolescent female programs.

Qualifications

The Lotus Case Manager / Intake Coordinator is responsible for
Managing Lotus calls, referrals and care coordination
Works with Lotus patients on any case management items that may come up
Responsible for attending off-site activities related to treatment needs
Works with clinical team to assist with communication with DCBS and discharge planning
Assists in discharge planning preparation as the liaison as patient is finishing programming materials
Assists with graduations on the unit and assists with the discharge plan when a patient is graduating or discharging from program
Demonstrates the ability to effectively communicate with community partners, clients and staff
Demonstrates the ability to prioritize the multiple demands of the position and is able to accurately follow instructions
Understands the documentation requirements and completes these requirements in a timely fashion
Excellent judgment and clinical skills are essential for this position
EDUCATION and/or EXPERIENCE: Graduate from a four year college or university with a degree in a clinical field. An equivalent combination of education and experience may substitute
