Time: 7:00 AM – 3:00 PM
$50 – $58 per hour
"Navitas Healthcare, LLC" is seeking an experienced and detail-oriented Registered Nurse Case Manager to support case management, utilization review, and clinical documentation improvement (CDI) functions within an acute care hospital environment. This role requires strong clinical judgment, effective collaboration with interdisciplinary teams, and proficiency with electronic medical records, preferably EPIC.
The ideal candidate will have recent acute care hospital experience and the ability to work independently in a fast-paced, regulated healthcare setting.
Conduct case management and utilization review to ensure appropriate patient care and effective use of healthcare resources
Review patient records for accuracy, completeness, and compliance with documentation standards
Collaborate with physicians, nursing staff, and interdisciplinary care teams to coordinate care plans
Monitor patient progress and facilitate discharge planning to ensure timely and safe transitions of care
Communicate effectively with patients, families, and healthcare team members
Ensure adherence to hospital policies, regulatory guidelines, and quality standards
Support clinical documentation improvement (CDI) initiatives to enhance documentation accuracy and reimbursement
Maintain accurate records and prepare required reports
Active New York State Registered Nurse (RN) License
BLS Certification (must be current and maintained)
Minimum 1.5 years of recent experience (within the past 2 years) in an acute care hospital setting of similar size and complexity
Experience using EPIC Electronic Medical Records (preferred)
Strong communication, organizational, and interpersonal skills
Experience in Clinical Documentation Improvement (CDI)
Acute care hospital or trauma center experience
