Home at the Heart Group Living is seeking an experienced Supported Living Program Coordinator to oversee and coordinate services for adults with intellectual and developmental disabilities receiving Supported Living services.
The Program Coordinator will be responsible for program operations, staff supervision, service coordination, documentation compliance, community integration, and communication with Support Coordinators, families, and community partners. This position plays a key role in ensuring services are person-centered, compliant with DIDD requirements, and focused on helping individuals achieve their goals and maintain independence.
Essential Duties and Responsibilities
Coordinate Supported Living services for individuals receiving IDD supports.
Develop and maintain positive relationships with Support Coordinators, families, guardians, and community partners.
Monitor implementation of Individual Support Plans (ISPs).
Supervise and support Direct Support Professionals (DSPs).
Assist with hiring, training, and onboarding staff.
Review daily documentation and service records for accuracy and compliance.
Conduct monthly quality assurance reviews.
Participate in team meetings and ISP meetings.
Ensure individuals receive services that promote independence, choice, and community inclusion.
Assist with admissions, transitions, and service planning.
Respond to incidents and ensure required reporting is completed.
Assist with scheduling and staff coverage as needed.
Maintain compliance with DIDD regulations and agency policies.
Qualifications
High School Diploma or GED required.
Associate's Degree or Bachelor's Degree preferred.
Minimum of 2 years experience working with individuals with intellectual and developmental disabilities.
Minimum of 1 year experience in Supported Living, Residential Habilitation, Case Management, Program Coordination, House Management, or a supervisory role.
Knowledge of DIDD regulations and person-centered supports preferred.
Strong organizational, communication, and leadership skills.
Valid Driver's License and reliable transportation.
Ability to pass all required background checks and screenings.
