What You Bring
Minimum: HS Diploma + 12 college credits in a related field
Preferred: Bachelor’s degree in psychology, social work, counseling, or related field
1+ year of direct client care experience (internships/volunteer work considered)
Experience with mobile services and case management preferred
Strong communication and computer skills
Valid Driver’s License
Ability to meet physical requirements of the role, with or without accommodation

Your Role
Provide community-based Blended Case Management services to children and adults in Central Bucks County
Meet individuals and family members in their homes and communities to assess needs and connect them to vital services, resources and programs to positively impact an individual’s overall well-being
Collaborate with LVF’s multidisciplinary team and external providers
Maintain accurate, recovery-oriented documentation in our Electronic Health Record (EHR)
