Fresno, CA Working Hours/Shift:
Mondays - Thursdays 10 hour AM Shift, Off Fridays. Working hours are approximate and may include flexibility as needed to meet client needs.

What you bring to SBHG:
Education & Experience: One of the following pathways is required:
An associate degree in a social service field plus two (2) years of experience in a mental/behavioral health setting. Or
A bachelor’s degree in a social science field plus six (6) months of experience (preferred).
Note: Relevant internship experience may be substituted for a portion of the required work history
Licensure & Compliance:
A valid California Driver’s License and eligibility to meet agency driving criteria.
How you will make a difference:
Provides services at the office, in the community, and in clients’ homes.
Uses a personal vehicle to drive to and from off-site locations to provide services. May be required to transport clients and/or family members in a personal vehicle (mileage reimbursement included)
Provides case management services to clients and their families, as deemed necessary and appropriate through the assessment process and in collaboration with the treatment team.
Participates in meetings, such as Treatment Team Meetings, to ensure clients’ needs are met and recommendations are followed through.
Documents all services provided clearly and concisely within the Electronic Medical Record within the timelines and framework established by Stars. Assists the primary therapist with initial and ongoing assessments of the client and family to identify strengths and needs.
Monitors the clients’ progress toward goals outlined in their treatment plans. Informs the primary therapist of progress.
Monthly stipend included (Cell phone / Internet)
