About the Role The Case Manager is responsible for coordinating and delivering comprehensive care management services to clients receiving behavioral health treatment. This role focuses on assessing client needs, developing individualized service and discharge plans, facilitating access to internal and community resources, and ensuring continuity of care.
The Case Manager serves as a liaison between clients, families, clinical staff, healthcare providers, and community agencies. Through advocacy, care coordination, and crisis support, the Case Manager helps clients achieve treatment goals, improve functioning, and maintain long-term stability.
Minimum Qualifications Minimum of two (2) years of experience in case management, social services, or behavioral health
Knowledge of mental health and/or substance use disorders
Understanding of community resources and referral processes
Experience with crisis intervention and de-escalation
Strong organizational and documentation skills
Excellent verbal and written communication skills
Preferred Qualifications Bachelor’s degree in Social Work, Psychology, Human Services, or related field
Certification in Case Management (CCM) or equivalent credential
Experience in a behavioral health, substance use, or residential treatment setting
Familiarity with Florida DCF and/or Joint Commission standards
Proficiency with electronic health records (EHR)
Responsibilities Conduct comprehensive biopsychosocial and needs assessments
Develop, implement, and update individualized service and discharge plans
Coordinate care with therapists, medical providers, families, and external agencies
Facilitate referrals to community resources (housing, employment, medical, legal, etc.)
Provide crisis intervention and support as needed
Monitor client progress toward treatment and discharge goals
Maintain accurate, timely, and compliant documentation in the EHR
Participate in multidisciplinary treatment team meetings
Advocate for client needs and support client engagement in treatment
Ensure compliance with facility policies and regulatory standards
