Mental Health Kokua (MHK) is hiring a CIS Homeless Outreach Case Manager to join our East Hawaiʻi team in Hilo . This field-based role focuses on engaging adults experiencing homelessness and serious mental illness and connecting them with housing, healthcare, and essential community services.
As a Case Manager, you will build relationships with individuals in the community, coordinate services with partner agencies, and help clients overcome barriers to stability and recovery. This role is ideal for someone who enjoys outreach work, problem-solving, and advocating for people who need support the most.

What You’ll Do (Responsibilities)
• Client Engagement: Conduct field-based outreach to build trust with adults experiencing homelessness and serious mental illness
• Caseload Management: Maintain a caseload of up to 15 clients and meet with each client regularly to support service goals
• Assessment & Referrals: Assess needs and coordinate referrals for psychiatric, medical, and housing services
• Service Coordination: Assist clients with obtaining identification, benefits enrollment, and health plan registration
• Care Collaboration: Participate in case conferencing with Continuum of Care partners and service providers to support housing placement
• Documentation & Reporting: Complete assessments, service plans, progress notes, and reporting in accordance with program requirements
• Data Management: Enter and maintain client data in HMIS and other required systems
• Insurance Verification: Confirm insurance coverage and payer authorizations as required
• Team Participation: Attend supervision, trainings, and team meetings to ensure quality, person-centered services

CIS Homeless Outreach Case Manager Qualifications
Minimum: High school diploma or equivalent, plus 3 years of experience in human services, behavioral health, or social services
Preferred: Bachelor’s degree in Social Work, Psychology, Human Services, or related field; experience supporting individuals with co-occurring mental health and substance use challenges
Current TB clearance, CPR, and First Aid certification (or ability to obtain)
Valid Hawaiʻi driver’s license, vehicle insurance, safety inspection, and clean driving abstract
Must pass a criminal background check and drug screening
Able to lift up to 25 lbs and travel throughout for field outreach

Field-based outreach work throughout East Hawaiʻi and surrounding communities
Flexible schedule may include non-traditional hours to effectively engage clients
Work involves driving, walking, and meeting clients in community settings
