About the Role:
As a Clinical Liaison, you'll play a vital role in:
- Patient Screening: Conduct preadmission assessments to ensure candidates meet IRF criteria, including medical stability, therapy needs, and insurance eligibility.
- Care Coordination: Collaborate with physicians, case managers, and hospital teams to facilitate seamless transfers to our inpatient rehab facility.
- Education & Advocacy: Educate patients and families on the benefits of intensive rehabilitation and guide them through the transition process.
- Marketing & Outreach: Build strong relationships with referral sources such as acute care hospitals, physicians, and case managers while promoting Oasis Point's advanced rehabilitation services.

Ideal Candidate:
- Licensed RN, PT, OT, or SLP with hands-on experience in IRFs or acute care settings.
