Summary of Job Description:
The Community Health Program Coordinator is responsible for coordinating, implementing, and evaluating community-based health and wellness programs that support the health priorities of the Mohegan Tribe. This position works collaboratively with Health Department staff, Tribal departments, community partners, and Tribal members to develop and deliver programs focused on chronic disease prevention, nutrition, physical activity, maternal and child health, agriculture, and overall wellness.
The coordinator will assist with program planning, participant engagement, data collection, grant reporting, event coordination, and community outreach while ensuring programs are culturally relevant, responsive to community needs, and aligned with departmental and grant objectives.

Minimum Requirements:
Demonstrated strong organizational and project management skills
Demonstrated strong and effective interpersonal communication skills
Ability to work independently and manage multiple priorities
Exceptional customer service skills
Comfortable interacting with community members of all ages
Proficient in Microsoft Office programs, including Excel, Word, Outlook, and PowerPoint
Ability to work flexible schedule, including evenings and weekends, and travel occasionally in the local and non-local regions
Passionate about public health, community wellness, and working in a culturally grounded environment
Ability to sit, stand, and walk for extended periods
Ability to lift and/or move up to 25 pounds, including pushing/pulling, carrying, crouching and reaching
Associate’s degree in Public Health, Health Education, Community Health, Human Services, Nutrition, Family Studies, Social Work, or related field; bachelor’s degree preferred
CPR/First Aid certification (or willingness to obtain)
Two (2) years of experience in community health, public health, health education, wellness programming, social services, or related field
One (1) to two (2) years’ experience coordinating and conducting programs, projects, events, or community initiatives
One (1) to two (2) years’ experience with data collection, tracking, and reporting
One (1) to two (2) years’ experience with program evaluation and performance measurement
Familiarity with Tribal communities and culturally grounded programming, preferred
Knowledge of chronic disease prevention, nutrition, physical activity, maternal and child health, or community wellness initiatives, preferred
Experience supporting grant-funded programs and reporting requirements, preferred
Experience conducting community outreach and participant engagement activities, preferred
Successful candidate must pass the background check in accordance with the Indian Child Protection and Family Violence Prevention Act (ICPA)
No felony convictions or misdemeanors involving moral turpitude or issues of integrityValid driver’s license from state of resident
