The Case Management Department collaboratively works with the social workers,
physicians, nursing and other health care disciplines to render the highest
quality of health care to the patients at the appropriate setting and to ensure
a timely, safe and seamless transition to the next level of care in the most
cost-effective manner..
Qualifications
QUALIFICATIONS:
Education Required: Bachelor’s Degree in human services / related discipline
required. Master’s Degree in preferred
License(s) Required: None
Experience/Skills
* A minimum of two (2) years of experience in case management is required.
* Prior experience in the Health Home model preferred.
* Excellent communication skills, verbal and written
* Working knowledge of physical and/or mental health
* Ability to travel to meet with patients
* Proficiency in electronic data collection systems
* Bilingual Spanish/English preferred
