Housing Case Manager - Twin Cities
Pay Range: $25.00 - $27.00 per hour
Schedule: Daytime hours, 3-day workweek (10 hours per day) or 4-day workweek (8 hours per day)

Housing Case Manager Duties:
Provide comprehensive housing case management services to recipients of housing subsidy vouchers, ensuring individualized support and advocacy throughout the housing process.
Assist clients in identifying, securing, and transitioning into safe, stable, and affordable housing options.
Collaborate with individuals to identify and address barriers to housing placement and retention through proactive problem-solving and resource coordination.
Conduct assessments to determine additional support needs related to obtaining and maintaining housing.
Promote long-term housing success by connecting clients with community resources and supportive services, including case management, utility assistance, and county or public benefits programs.
Housing Case Manager Requirements:

Valid driver's license, vehicle insurance, and acceptable driving record required
Experience working with individuals who have experienced long-term homelessness is highly desired
Experience partnering with community resources and supportive services in the Twin Cities highly desired
Experience working with individuals affected by mental health and/or chemical dependency issues is desired
