Schedule: Monday – Friday (Rotational Saturday with Flexible Hours)

About The Role

The Life Center Case Manager plays a vital role in supporting individuals on their journey toward recovery and independence. This compassionate and dynamic position combines instruction, advocacy, and case management to help participants build daily living skills and achieve personal goals. Through structured interventions, educational sessions, and coordinated care, the Case Manager fosters a supportive and empowering environment at The Life Center, ensuring each participant receives the guidance and resources needed.

Key Responsibilities

Client Orientation & Engagement

Welcome and orient new clients to the program.

Provide clients with a copy of their rights upon enrollment and annually.

Build and maintain rapport to foster trust and therapeutic engagement.

Assessment & Treatment Planning

Obtain a biopsychosocial assessment from a licensed professional within 30 days of admission.

Ensure substance abuse evaluations are acquired when applicable.

Develop individualized treatment and rehabilitation plans based on assessed needs.

Assist clients in understanding and processing their diagnoses.

Service Delivery & Instruction

Plan, coordinate, and facilitate day rehabilitation services.

Lead and model life skills development through structured learning groups.

Provide coaching and support for independent and community living skills.

Assist in planning and participating in evening and weekend social activities.

Advocacy & Case Management

Support clients in accessing external services such as medical, dental, and legal resources.

Promote agency services and foster partnerships with community resources.

Provide transportation to clients when necessary.

Conduct outreach and intervene in crises as needed.

Program Compliance & Monitoring

Monitor and enforce adherence to the medication provision policy.

Follow food service policies to ensure compliance and safety.

Ensure all services are delivered in alignment with agency policies and procedures.

Maintain accurate and timely documentation for all clients interactions.

Quality Assurance & Continuous Improvement

Participate in internal quality assurance efforts and audits.

Contribute to ongoing evaluation and improvement of program services.

Collaborate with team members to ensure high standards of care.

Professionalism & Ethics

Maintain professional boundaries and ethical conduct at all times.

Demonstrate flexibility and openness to feedback and new practices.

Exhibit strong oral and written communication skills.

Actively participate as a member of the interdisciplinary team.

Perform other duties as assigned by the supervisor or program leadership.

Qualifications

Option 1: Bachelor’s degree or higher in a human services-related field.

Option 2: High school diploma with at least two (2) years of experience working with individuals with a mental health diagnosis.

Required Skills & Attributes

Strong knowledge of counseling principles and mental health support practices.

Ability to provide structure, set limits, and support behavior change.

Skilled in teaching and modeling life skills and appropriate behaviors.

Ability to form therapeutic relationships and work collaboratively with staff.

Effective communication, organization, and documentation skills.
