About Sentido Health
Sentido Health is a boutique home health solutions company founded on a deeply personal mission — to support medically complex patients and their families with the heartfelt care we'd give our own. We offer the product selection and clinical resources of large national organizations, delivered with the individualized approach only a boutique provider can. We consider our patients family, and our team is built on the belief that every person deserves more than a delivery — they deserve a partner.
The Opportunity
We're not just offering a job — we're offering a meaningful career in a mission-driven environment.
As a Nutrition Clinical Liaison, you'll play a critical role in expanding access to care for our medically fragile patients while supporting referral partners and driving territory growth. You'll join a collaborative, high-performing team that values clinical excellence, relationship-building, and measurable results. We're looking for a motivated Registered Dietitian who thrives in a fast-paced, field-based environment and is passionate about improving patient outcomes through education, advocacy, and strategic account management. Bilingual candidates are strongly encouraged to apply.
What You'll Do
- Develop and grow a territory by building strong, lasting relationships with physicians, discharge planners, social workers, and other key referral sources
- Consistently meet and exceed monthly sales revenue targets and territory conversion goals
- Identify new business opportunities within existing accounts and across new call points
- Serve as a concierge-level representative for Sentido, managing all key account communications with professionalism and urgency
- Facilitate coordination of care between referral sources — including hospitals, physician offices, and home health agencies — to ensure smooth patient transitions
- Conduct initial and ongoing patient and caregiver education on nutrition therapy services, equipment, and technology
- Assess patient needs based on physician orders and introduce formulary options, route-of-feeding recommendations, and advances in nutrition support
- Build and execute strategic weekly call routes to maximize face-to-face account visits
- Manage monthly/quarterly territory budgets (fuel, travel, marketing, meals) to maintain profitability
- Maintain after-hours on-call coverage to ensure patients always have support – as required
- Respond to same-day emergency deliveries or home visits as needed
- Handle all patient health information, sales messaging, and marketing materials in full compliance with HIPAA and company guidelines
- Participate in educational meetings, product trainings, and company events
What You Bring
- Proven ability to build referral relationships and drive new business in a healthcare setting
- Sales-driven mindset with a genuine passion for patient outcomes
- Excellent verbal and written communication — equally effective in a physician's office and a patient's home
- Self-starter who thrives in a fast-paced, field-based, independent environment
- Strong organizational, time management, and prioritization skills across a full territory
- Solutions-oriented problem solver with sound clinical judgment
- Ability to educate patients and families on equipment and services with clarity and empathy
- High professional standards: integrity, accountability, and follow-through are non-negotiable
