Registered Nurse (RN) Case manager– Adult Residential Services Make a difference every day.
Fairbanks Resource Agency (FRA) is seeking a compassionate, organized, and motivated Registered Nurse case manager to join our Adult Residential Services team. In this role, you'll help adults with intellectual and developmental disabilities receive high-quality, person-centered healthcare while supporting them to live safely and independently in the community.
As our Adult Services Nurse, you'll work closely with individuals receiving services, direct support professionals, guardians, healthcare providers, hospitals, and community partners to coordinate medical care, develop individualized plans, and advocate for each person's health and well-being.

What You'll Do Coordinate and oversee medical services for individuals receiving residential supports.
Develop, implement, and update individualized Plans of Care.
Complete nursing assessments and monitor participant health needs.
Coordinate medical appointments, specialty care, hospital discharges, and follow-up services.
Prepare and submit Medicaid waiver documentation and required renewals.
Track important medical deadlines, including disability renewals, immunizations, and annual assessments.
Provide nursing consultation and training to Residential Site Supervisors and Direct Support Professionals.
Conduct home visits and routine participant wellness monitoring.
Maintain accurate documentation within the electronic medical record (EMR).
Collaborate with physicians, hospitals, Senior and Disabilities Services, guardians, care coordinators, and other community partners.
Participate in interdisciplinary team meetings and advocate for participant needs.
Assist with direct care and skills training as needed.
Maintain compliance with agency policies and all applicable state and federal regulations.
Participate in required trainings, staff meetings, and professional development.
Perform other duties as assigned.
Qualifications Required
Current Alaska Registered Nurse (RN) license.
Valid Alaska driver's license.
Ability to meet agency driving and insurance requirements.
Ability to travel throughout the community for home visits and appointments.
Strong communication, organization, and documentation skills.
Experience using electronic medical records or other healthcare software.
Preferred
Two years of nursing experience supporting adults with intellectual or developmental disabilities.
Public Health nursing experience.
Experience with Medicaid Waiver services or care coordination.

Is proficient with computers and willing to learn agency software and electronic medical record systems.
Certifications The following certifications are required or must be obtained upon hire in accordance with agency requirements:
Alaska Registered Nurse License
CPR (Adult and Infant/Child)
First Aid
Safety-Care Certification
Immunization Requirements Annual TB screening (PPD or approved alternative)
Hepatitis B vaccination or signed declination/waiver
Physical Requirements This position requires occasional lifting of up to 50 pounds and frequent movement while assisting participants in residential settings. Duties include standing, walking, driving, documenting electronically, and communicating effectively with participants, staff, and healthcare providers.
