The CDO Support Broker/Case Manager is responsible for coordinating services for individuals that have selected Consumer Directed Options, a choice under the Supports for Community Living, Michelle P. and Acquired Brain Injury waivers. The CDO Support Broker is a positive team player that promotes a culture of inclusion, and participation.
Required Education and Experience
Bachelor’s degree in a human services field.
Valid driver’s license, reliable operating transportation, and proof of current auto insurance
Preferred Education and Experience
Experience working with IDD population and Medicaid Waiver programs
SCL (Supports for Community Living) Intellectual Disability Professional
Essential Functions

Case Management
Complete assessments to determine eligibility for ongoing services
Maintain client plan of care that is reflective of needs as identified by interdisciplinary team
Develop crisis prevention plan and connect participants with additional community-based services to help prevent institutionalization
Process employee paperwork and coordinate training prior to service provision
Complete required documentation and training
Participate in and promote performance improvement activities
Cost Containment and Productivity
Work collaboratively with other region-wide programs to determine services offered in the Region
Ensure that staff and other resources within the Region are utilized in a manner that optimizes the service delivery system
Corporate Involvement and Responsibilities
Actively participate in corporate-wide initiatives
Support and work within the mission and strategic plan of the organization
Ensure compliance with relevant laws, regulations and standards. e.g., state contracts, Joint Commission, etc.
