Position
Reporting to the Children's Programs Manager, this position is responsible for case management work primarily consisting of administration and partnering with young adults served by Palmer Home. The Transitional Care Case Manager is responsible for providing coaching and tools for participants to become independent.
Responsibilities
Develop and maintain positive working relationships with young adults and those engaged in the life of the young men and women as well as other program employees .
Provide ongoing communication with participants and support teams working with young adults toward independence and self-sufficiency.
Make monthly visits to program participants to monitor and provide coaching according to goals outlined in the participants' I ndividual Service Plan .
Make weekly contact by phone or virtually with participants to assess goals and maintain ongoing communication.
Develop and effectively communicate the documented case plans , recommendations,
and maintain
contact with young adults in the program as well as caregivers, service providers, educational support members , and agencies.
Review and manage case strategies for each participant to ensure that the objectives of individualized service plans are achieved, regularly communicating those results to the Children's Programs Manager .
Participate in ongoing employee training as required .
P rovide coaching to participants on medical appointments, scheduling, records, finances, education, and employment.
Evaluate and work with participants to ensure that they maintain a safe and consistent environment for themselves to achieve independence.
Work fully within the philosophy, goals and operating procedures of Palmer Home for Children demonstrating exemplary Christian character.
Qualifications
Bachelor’s degree in social work or related field and min of three years’ experience in case work capacity.
A ctive Social Work license preferred
Knowledgeable and willingness to work alongside local social services, agencies and programs with which Palmer Home must frequently interact to provide additional services to our young adults.
Strong interpersonal skills.
Strong computer skills
Must have exceptional skills in program administration, communications, and human relations.
Willing to travel as required or needed to successfully accomplish requirements of this position
