Job Summary
Reporting to the Director of Social Services/Social Services Supervisor, the Case Manager provides and documents clinical and case management services for assigned clients. The Case Manager will be an active listener and provide high quality customer service and respond by exhibiting cross-cultural awareness with the ability to promote and contribute to an equitable and inclusive anti-racist organizational culture and environment.
Key Responsibilities & Essential Functions
Program/Facilities
Provides comprehensive case management services for assigned clients including, but not limited to:
Support and resources/referrals for clients to meetobjectives, including but not limited to:
Entitlements
Housing
Employment / Education / Vocational Training
Health, mental health treatment, and substance abuse services
Social and community integration
Counseling and support to clients on an on-going basis including, but not limited to:
Crisis intervention
Training and support on client daily living skills, asrequired
Assistancein problem solving, asrequired
Conflict resolution
Advocacy
Assesses client needs, develops service plans and/or service referrals to achieve client self-sufficiency.
Conducts field work including, but not limited to home visits, escorting clients to appointments, and outreach.
Facilitates client groups.
Monitors client medications, asrequired.
Conducts client intake and updates information in client database system(s).
Completes incident reports anddistributes related notices within 24 hours.
Administrative
Maintains andsubmitsall necessary documentation and reports accuratelyin a timely mannerand as required by Urban Pathways, regulatory and funding agency policies, and procedures including but not limited to:
Enterprogress notes after each client meeting within48 hoursand/or as mandated bytheProgram.
Completes Intake/Admission documentation within 24 hours of client admission, as mandated bytheProgram.
Documentsassessments in client records withinrequiredtimeframe(not applicable for Outreach Program).
Updates information in client records, asrequired.
Documentsincidents and distributes related notices within 24 hourswhen the Case Manager haswitnessedthe incident.
Submitall requiredpaperworkfor supervisor approval within48hours.
Files paperwork inclientcharts(once approved/signed by supervisor), as required by program (no more than 48 hours).
Community Relations
Develops andmaintainsrelationships with community services and resources with particular emphasis on substance/alcohol abuse programs, housing, health and mental health programs,employment,and benefits.
Attends andparticipatesin external meetings with fundingagencyand housing providers.
Communication
Communicates verbally and in writing in a professional manner at all times.
Attends staff meetings, trainings, and supervisory sessions, asrequired.
Responds to all communications including telephone,email,and other oral and written inquiriesin a timely manner.
Performs all other duties as assigned and as required.
Qualifications, Education and Experience
Bachelor’sdegreerequired. Bachelor’s degree in Social Workorotherrelated field preferred.
Minimum one year of case management, individual or group counseling experience preferred.
Knowledge of NYC housing subsidy programs.
Experience with people experiencing homeless, mental illness and/orsubstance abuse population preferred.
Skills & Competencies
Proficiencyin Microsoft Office (e.g.Word, Excel, Outlook, etc.) and electronic record-keeping systems (e.g.Awards, Cares, Caires, etc.).
Ability tomaintainaccurateand up-to-date case notes, documentation, and records.
Excellent organizational and time management skills.
Excellent communication,interpersonalabilities, and follow-up skills.
Work effectively with individuals from diverse backgrounds and cultures.
Foreign languagesa plus.
Other
Required toworka flexible schedule including weekends, holidays, and evenings.
Capabilities
Desire to perform the essential functions of this job.

Licenses
Valid driver’s licensewithclean driving recorda plus.
Physical Requirements
All programs requirefieldwork, home visits, client escorts, and use of public transportation.
